In an effort to ensure that our schools and communities remain safe for students and staff, COVID-19 testing opportunities may be offered to students throughout the school year. This testing will be supported by Michael Garron Hospital in collaboration with Toronto Public Health, the school boards and the Ministry of Education.
Students and staff may be recommended for COVID-19 testing based on the following scenarios:
Getting tested is an essential part of keeping our schools and communities safe and we are seeking your consent to test your child at school for COVID-19 if required.
Method of testing:
Testing will be conducted using an easy and simple mouth/nose swab, which involves rubbing the inside of the mouth and front of the nose for less than 5 seconds. Students can self-administer the swab if they feel comfortable doing so. In some scenarios, the team will provide instructions to support sample collection at home. I have read the provided COVID-19 testing information and understand that testing may occur at my child’s school. By completing this form, I consent to my child receiving a COVID-19 test.
