COVID-19 Vaccine: First Dose at Home Program and Homebound Visits

Michael Garron Hospital (MGH) and East Toronto Health Partners (ETHP) are visiting homebound individuals and seniors in East Toronto to offer the COVID-19 vaccine.

Our homebound vaccination visits help individuals who are homebound receive their first, second, third (booster) and/or fourth (booster) dose of the COVID-19 vaccine. To be eligible for a homebound vaccination visit, individuals must:

  • Be 65 years of age or older; and/or
  • Have medical conditions or mobility issues; and/or
  • Have difficulty or challenges leaving their home or visiting a local vaccine clinic,

AND

  • Need their first, second, third (booster) and/or fourth (booster) dose of the COVID-19 vaccine,

AND

  • Live in a postal code beginning with M1L, M3C, M4A, M4B, M4C, M4E, M4H, M4J, M4K or M4L.

If you are requesting a fourth (booster) dose, you must have received your third dose more than 140 days ago.

Individuals who meet the above criteria are invited to submit a request using the form below. A member of our team will respond within 7 calendar days. You can also call the MGH COVID-19 vaccine booking office at 437-537-5131 for help requesting a homebound visit.

Individuals who receive a confirmed homebound visit from our team will be able to receive their first, second, third (booster) or fourth (booster) dose. In addition, one essential caregiver who provides care to the homebound individual may also receive their first, second, third (booster) or fourth (booster) dose during our team's visit. Pfizer or Moderna will be administered.

Due to workforce capacity, homebound visits will be scheduled approximately 3-4 weeks after the request. If you have someone who is able to assist you in getting to a vaccine clinic before this time, we strongly encourage you to do so. You can book an appointment for your first, second, third (booster) and/or fourth (booster) dose at a local clinic using the provincial booking system or by calling 1-833-943-3900. You can also book an appointment at a participating pharmacy

Our First Dose at Home Program helps seniors in East Toronto receive their first dose of the COVID-19 vaccine. To be eligible for this program, individuals must:

  • Be 65 years of age and older;
  • Have not received a first dose of the COVID-19 vaccine; and
  • Live in a postal code beginning with M1L, M3C, M4A, M4B, M4C, M4E, M4H, M4J, M4K or M4L.

Eligible individuals can self-refer to the First Dose at Home Program. You can also refer your family members, friends, neighbours and others! Pfizer or Moderna will be administered.
