The Virtual Ward provides support and care for patients in their homes after they’ve been discharged from hospital. We provide regular phone calls, perform medical reconciliation and arrange follow-up appointments. We liaise with home care and community partners to keep the updated on progress and suggestions. We work together with patients and families to help navigate the healthcare system and available supports.
