Overview

The Nurse Practitioner (NP) - Geriatrics is a nurse who is prepared at the graduate level and is registered with the College of Nurses of Ontario in the Extended Class. They have experience and knowledge about seniors' health and will provide direct pat ient care that focuses on the specialized needs of older adults. The NP possesses the competencies to autonomously diagnose, order and interpret diagnostic tests, prescribe pharmaceuticals and perform specific procedures within his/her legislated scope of practice. The NP Geriatrics uses in-depth nursing knowledge, clinical expertise, research and educational skills to support the care of the older, frail and complex adults receiving care at Michael Garron Hospital (MGH). The NP-Geriatrics will work i n collaboration with physicians and other medical staff and members of the interdisciplinary team as detailed in the NP’s Collaborative Practice Agreement.

In addition to clinical practice, this is a formal clinical leadership role that will work wi th stakeholders across the organization as NP Director of MGH’s Senior’s Strategy. The NP Geriatrics will lead this essential clinical strategy to enhance the quality of the patient experience for one of our most vulnerable patient populations.

T he NP will:
- Assess and treat patients admitted under the Geriatrics program within the Medicine Health Service at Toronto East General Hospital, providing care for patients within their legislated scope of practice.

- Work in partnership with stakeholders, physicians, nurses and other inter-professional members to develop a culture of collaborative patient centered care and to ensure safe care transitions for clients and families.

- Demonstrate clinical excellence through all four domains of NP practice: clinical care, leadership, education, and research.

- Provide coaching and mentoring to staff that enhances the provision of quality care using evidence based practices and research.

Education

- Master in Nursing / Nurse Practitioner (MN-NP) or Masters of Science in Nursing with successful completion of a post Master's Acute Care Nurse Practitioner Certificate Program from a recognized school of graduate studies required- Adult Stream
- Cur rent College of Nurses of Ontario registration in the Extended Class (EC) as an NP (adult) or NP (primary health care) required
- CNA Certification in Geriatrics would be an asset
- BLS required
- Additional specialty education in Gerontology pr eferred
- Clinical specialty organizational membership: RNAO (required)

Qualifications

- 1-3 years of demonstrated experience as a Nurse Practitioner required
- 3-5 years of relevant acute medicine or Primary Care experience specializing in Seniors health
- Demonstrated sound knowledge and skill with adult education approaches and me thods
- Demonstrated in-depth expertise regarding clinical assessment and acute care needs of vulnerable elderly
- Demonstrated ability to work in a self-directed manner with well developed leadership, critical thinking, organizational and problem so lving skills
- Demonstrated interpersonal, team development, and conflict management skills
- Advanced-level knowledge of patient safety, change leadership, project management and evaluation an asset
- Demonstrated strong communication and interpe rsonal skills, with the ability to work collaboratively with physicians, inter-professional team members, and other stakeholders
- Possesses highly developed analytical, problem solving and critical thinking skills
- Excellent oral, written, and pre sentation skills, with demonstrated competency in information and communication technology
- Demonstrated excellent organizational and coordination skills and the ability to prioritize multiple competing workload demands
- Demonstrates an ability t o provide leadership, support and critical analysis in stressful, complex clinical and emotional situations
- Experience in Case Management preferred and knowledge of community resources.
