This position is responsible for the provision of clinical social work services to hospital patients, families and providing excellent patient care within an interdisciplinary team setting. Responsibilities include: providing comprehensive assessment and treatment planning, educational and supportive counseling, resource identification, referral and linking, and collaborating and consulting with inter-professional team and community based service providers.
- Must be a member of the College of Registered Social Workers and Social Service Workers, or be a candidate for certification
- Master of Social Work from an accredited school of social work, or equivalent degree, as recognized by the Colleg e of Registered Social Workers and Social Service Workers
- Knowledge and / or experience in both Emergency/Crisis and Maternal Newborn environments required
- Working knowledge of the Children's Aid Society required
- Knowledge of ALC framework; knowledge of and experience working with CCAC
- Ability to dev elop and maintain positive working relationships with social work staff and staff from other disciplines
- Keen attention to service quality and innovation
- Demonstrated ability to provide comprehensive assessment and treatment planning
- Demonstrate d ability to provide educational and supportive counseling
- Demonstrated ability to provide resource identification, referral and linking
- Demonstrated ability to provide collaboration and consultation with inter- professional team and community base d service providers
- Ability to participate in education to various groups including social work and other health professionals, students, patients and families, colleagues and community agencies
- Ability to participate in program and hospital develo pment, planning, implementation and evaluation of service
- Strong written and verbal interpersonal and communication skills required
- Experience in Maternal/Newborn/Pediatrics preferred
- Demonstrated ability to prioritize and work effectively in af ast paced environment
- Demonstrated family centered philosophy of care provision
- Demonstrated ability to welcome change and manage it innovatively
- Good work and attendance record required
