Overview

The MGH Home Peritoneal Dialysis (PD) Program promotes self-management of the independent dialysis patient who has chosen have treatment at home. The care provision is a blend of in home and in hospital, with a multidisciplinary approach. The Registered Nurse on this team has a unique opportunity to practice autonomously in a dynamic professional environment while facilitating and coordinating the patient's kidney care journey. The PD nurse will be responsible for conducting assessments of patients’ ap propriateness for PD, conduct PD education and training for patients and their families using adult learning principles, conduct home visits, clinic follow up, PD catheter management, assistance with transplant workup, support with data collection and ot her tasks as required. Shared on call responsibilities are required.

Registered Nurses (RNs) are accountable to the professional standards of nursing practice from the College of Nurses of Ontario. Kidney Care RNs possess a defined body of advanced r esearch-based knowledge, specialized clinical skills, procedural skills and professional collaboration that is directed at comprehensive, effective patient and family centered care. Kidney Care RNs integrate leadership competencies within the inter-profe ssional care team. RNs apply their clinical knowledge and leadership skills to collect and interpret information, make appropriate clinical nursing decisions and carry out therapeutic nursing interventions.

Education

- Registered Nurse with current Certificate of Registration in good standing with the College of Nurses of Ontario.
- BScN required.
- BCLS required.
- Completion and maintenance of Vascular Access Certification required.
- Nephrology Certificat e or evidence of working towards required.
- CNA certification in Nephrology an asset

Qualifications

- Must be able to work in the hospital environment as well is in the community (in the patients’ home) and able to travel from the hospital to patients’ homes in a personal vehicle or in transportation provided. - Minimum of 2 years recent hemodialysis experience and/or peritoneal dialysis experience required. Preference will be given to candidates with PD experience.
- Recent experience with the kidney patient population (ESRD, Diabetes and its complications) required.
- Current clinical experien ce working with PD devices an asset.
- 2 years recent clinical experience in providing care to patients with central venous access devices, or PD catheters including assistance with insertions/removals.
- 2 years recent clinical experience with a Hom e Peritoneal Dialysis Program, or Inpatient care of PD patients an asset.
- Demonstrated knowledge of current kidney care best practices.
- Demonstrated ability to teach patients and families, and apply adult learning principles.
- Strong leadershi p and collaboration skills and experience taking the lead in multidisciplinary teams.
- Demonstrated ability to implement plan of care including independent and interdependent nursing functions.
- Excellent communication and interpersonal skills
- Demonstrated ability to participate in unit quality improvement initiatives. Demonstrated ability to multi-task and prioritize to meet the care demands of the day
- Ability to work independently as well as a team environment.
- Demonstrated ability to de-escalate situations as they arise.
- Demonstrates appropriate use of the MGH Core Service Standards.
- Demonstrates a commitment to continued professional and personal growth and development through continuing education programs and self-direct ed learning opportunities.
- Ability to read, write, and communicate in English.
- Demonstrated computer literacy.
