Overview

Located on site at Michael Garron Hospital, the Virtual Care Registered Practical Nurse (RPN), in collaboration with the rest of the Virtual Care team is responsible for the coordination of all care related to patients that are enrolled in Virtual Care a nd Remote Monitoring Programs. Duties include but are not limited to initial assessment, patient monitoring, and discharging patients from the programs.

The RPN Virtual Ward counsels patients regarding symptom management, health education and system navigation.

Education

- Current active registration in good standing with the College of Nurses of Ontario

Qualifications

- Minimum 8 - 10 years relevant clinical specialty-related experience in Medicine or Surgery as a Registered Practical Nurse
- Exceptional clinical assessment skills
- Experience with remote monitoring and telephone assessments preferred.
- Demon strated ability to work in a collaborative model with multidisciplinary teams
- Demonstrated ability to take initiative and work autonomously.
- Demonstrated ability to analyse clinical information and make decisions
- Excellent verbal and written communication skills
- Proficient in electronic documentation and web based applications
- Ability to handle multiple issues simultaneously
