Located on site at Michael Garron Hospital, the Virtual Care Registered Practical Nurse (RPN), in collaboration with the rest of the Virtual Care team is responsible for the coordination of all care related to patients that are enrolled in Virtual Care a nd Remote Monitoring Programs. Duties include but are not limited to initial assessment, patient monitoring, and discharging patients from the programs. The RPN Virtual Ward counsels patients regarding symptom management, health education and system navigation.
